DownloadWhen completing your paperwork, please print clearly and leave any questions that do not apply blank. The first section, called Subscriber Information Profile, will be stored on your account for our telephone operator's reference. It is important to include your scheduled lunch times and a back-up contact number in case of emergencies. You may also want to consider providing your office manager's contact information. In this section, you can also indicate if you would like to receive daily call reports and specify your preferred method for routing calls during business hours. All information provided is confidential and will only be used for the purpose of providing exceptional service to your practice. If you have any questions, please don't hesitate to reach out to us.
Section B of your paperwork, called HIPAA Business Associates, asks for the names and telephone numbers of any covering physicians that are not affiliated with your practice. These physicians may have their own answering service with different methods of contact and a HIPAA Business Associate Agreement. Providing their names and phone numbers can improve the quality of service we are able to provide. If you need to add a new covering physician, you can easily do so by faxing their information to our 24-hour support center. We use this information to create a drop-down menu for our operators, allowing them to quickly access and prepopulate fields to speed up the process without sacrificing quality.
In the last portion of Section B, you can provide any special instructions you would like us to follow. Instructions for contacting providers within your practice are covered in the next section, Section C. If you have any questions, please don't hesitate to ask.
It's important to note that each provider in your office who we will be contacting directly must complete an individual provider profile form. This form should not be completed by call group members, whose information is recorded in Section B of your paperwork.
Section C of your paperwork should be completed by each individual provider who we will be contacting according to the instructions they have provided. In the first table, please clearly print the provider's full name, specialty, and preferred methods of contact. If you have a preferred vendor for secure messaging or alpha paging, please include their name, city, and state.
Your contact schedule is customized for your practice and is accessible to our operators through your individual account. It functions as your standing orders and can be adjusted based on the time of day or other circumstances as defined by you.
To complete your provider profile, please provide any additional notes in the final section, sign, and date the form. Thank you for your cooperation. If you have any questions, please don't hesitate to reach out to us.
To use the telephone answering service provided by The Message Center, it is necessary to finalize the telephone answering service agreement in full. In the first blank field, please print the full name of the client, which is considered the practice or owner of the practice. Then, include your price quote, list rate, and the name of the sales associate in the appropriate fields.
Please note that our agreement is month-to-month and requires a 30-day written notice to unsubscribe, unless you are within a free trial period. When reviewing the terms of payment section, be sure to sign and date the form and specify the date on which you would like the service to begin. Once you have completed and submitted the agreement, you will be finished. If you have any questions, don't hesitate to reach out to us.
